What is the CFI Program?
The Choices for Independence (CFI) program is New Hampshire's Medicaid-funded Home and Community-Based Services Waiver — designed to help seniors and adults with disabilities stay home instead of moving to a nursing facility.
Eminence Home Care Services guides you through every step, from eligibility to your caregiver's first paycheck.
1915(c) Medicaid HCBS WaiverStay Home. Stay Independent.
CFI allows participants to self-direct their own care — choosing trusted family and friends — and receive professional services right at home.
Care Recipient Qualifies
Must be 18+ with disability or 65+, meet Medicaid income/asset limits, and have clinical need for nursing-level care.
Choose Your Caregiver
Under CFI's self-directed option, the care recipient selects a trusted family member, friend, or neighbor as their paid caregiver.
Eminence Handles the Rest
We manage paperwork, nursing assessments, care plans, payroll, tax withholding, and ongoing compliance.
Caregiver Receives Payment
Medicaid pays the caregiver directly. The person receiving care owes nothing out of pocket for covered services.
CFI-Covered Services
Benefits are individualized based on each participant's care plan. Here's what the CFI program may cover for qualified individuals.
Personal Care Assistance
Help with bathing, dressing, eating, toileting, and mobility — performed with dignity and compassionate respect.
Homemaker Services
Meal preparation, laundry, light housecleaning, and grocery shopping to keep the home safe and comfortable.
Home Health Aide
Skilled and unskilled in-home aides providing hands-on assistance with health monitoring and personal care.
Adult Day Health Care
Supervised daytime programs offering social activities, health monitoring, and assistance with daily living.
Home Modifications
Safety upgrades: wheelchair ramps, grab bars, widened doorways, and fencing for individuals with wandering concerns.
Non-Medical Transportation
Rides to medical appointments, community activities, and essential errands to maintain independence.
Home Meal Delivery
Nutritious meals delivered directly to the home for qualifying participants, supporting daily health and independence.
Emergency Response (PERS)
Personal Emergency Response Systems — medical alert devices ensuring safety and rapid help when living independently.
Respite Care
Scheduled relief for primary family caregivers — preventing burnout and sustaining high-quality, long-term care at home.
CFI Eligibility Requirements
Eligibility applies to the person receiving care, not the caregiver. If they qualify, they choose who provides their care.
- New Hampshire resident aged 65 or older
- Ages 18–64 with an official Social Security disability designation
- Need help with 2+ Activities of Daily Living (bathing, dressing, eating, toileting, mobility)
- Monthly income under $2,982 (single) or $5,964 (couple) — 2026 limits
- Assets within NH Medicaid limits ($2,500 for individuals)
- Have or are applying for Medicaid (Medicare alone does not qualify)
- Prefer to remain at home rather than in a nursing facility
- CFI is not an entitlement — enrollment is limited (~5,400 slots/year). A waitlist may apply.
- A registered nurse conducts a Medical Eligibility Assessment (MEA) to confirm clinical need
- Medicare alone does not meet program requirements — Medicaid coverage is required
- Caregivers must be 18+ and legally eligible to work in the United States
- No special nursing certification is required for family or friend caregivers
- Planning strategies (MAPTs, Medically Needy Pathway) may help those slightly over income/asset limits
- Both spouses may qualify independently if each meets individual eligibility criteria
