    The Choices for Independence (CFI) program is New Hampshire's Medicaid-funded Home and Community-Based Services Waiver — designed to help seniors and adults with disabilities stay home instead of moving to a nursing facility.

    Nutritious meals delivered directly to the home for qualifying participants, supporting daily health and independence.

    Emergency Response (PERS)

    Personal Emergency Response Systems — medical alert devices ensuring safety and rapid help when living independently.

    Respite Care

    Scheduled relief for primary family caregivers — preventing burnout and sustaining high-quality, long-term care at home.

    Who Qualifies

    CFI Eligibility Requirements

    Eligibility applies to the person receiving care, not the caregiver. If they qualify, they choose who provides their care.

    • New Hampshire resident aged 65 or older
    • Ages 18–64 with an official Social Security disability designation
    • Need help with 2+ Activities of Daily Living (bathing, dressing, eating, toileting, mobility)
    • Monthly income under $2,982 (single) or $5,964 (couple) — 2026 limits
    • Assets within NH Medicaid limits ($2,500 for individuals)
    • Have or are applying for Medicaid (Medicare alone does not qualify)
    • Prefer to remain at home rather than in a nursing facility

    • CFI is not an entitlement — enrollment is limited (~5,400 slots/year). A waitlist may apply.
    • A registered nurse conducts a Medical Eligibility Assessment (MEA) to confirm clinical need
    • Medicare alone does not meet program requirements — Medicaid coverage is required
    • Caregivers must be 18+ and legally eligible to work in the United States
    • No special nursing certification is required for family or friend caregivers
    • Planning strategies (MAPTs, Medically Needy Pathway) may help those slightly over income/asset limits
    • Both spouses may qualify independently if each meets individual eligibility criteria

    CFI has a limited number of enrollment slots (approximately 5,400 per year). When slots are full, a waitlist forms. Starting the application process as early as possible is critical. Eminence helps you get your application in quickly and correctly to minimize any delays in receiving benefits.

    Yes. CFI services can be provided in the participant's own home, a family member's home, an adult family care home (adult foster care), or a residential care/assisted living facility. Note that while CFI covers services in these settings, it does not cover room and board costs at assisted living or residential care facilities.
