Eminence Home Care is proud to be an approved provider of Home and Community-Based Services (HCBS) through Pennsylvania’s Office of Developmental Programs (ODP) waivers. We serve individuals with intellectual and developmental disabilities (IDD) across Montgomery, Bucks, Philadelphia, Chester, and Delaware counties. Services Offered
Assisting individuals with skill-building and daily living activities both at home and in the community to promote independence and meaningful engagement.
Offering companionship and support for daily activities, including light tasks, mobility assistance, and social interaction, to improve overall well-being.
Encouraging individuals to engage in social, recreational, and educational activities within their communities to foster inclusion and meaningful relationships. (1:1, 1:2, 1:3 options)
Providing temporary care for individuals in their own home to ensure their needs are fully met, with little disruption to their daily schedules while natural support is unavailable.
Helping individuals maintain a clean, safe, and organized living environment by assisting with household tasks such as cleaning, laundry, and meal preparation.

Eligibility Requirements
To receive services through our IDD/ODP Program, individuals must:
- Be enrolled in one of Pennsylvania’s ODP waiver programs
- Consolidated, P/FDS, or Community Living Waiver
- Reside in Montgomery, Bucks, Philadelphia, Chester, or Delaware County
- Have an Individual Support Plan (ISP) in place with a Support Coordination Organization (SCO)

If you or a loved one may be eligible but haven’t started the enrollment process, our team is here to help. Our Central Intake Team helps individuals access our provider services—but support doesn’t always start there. We support families in navigating their county’s service system. We’ll guide you to the right resources, connect you with your local Administrative Entity, and answer your questions with care, clarity, and respect.
